Generally, if you have low dexterity enemies are more likely to notice you, even if they are facing in the opposite direction. This gives you an opportunity to sneak around and avoid being noticed by enemies. You can only perform basic attacks on enemies that are right in front of you, but special abilities can target anyone you can see. At any given time you are only able to see things that are directly to the front or side of you. The game innovates over most traditional rogue-likes in its facing system. Players are thus kept on their toes trying to avoid getting killed by a creature they may not be expecting.
